1. In compliance to the Annual Physical Examination (APE) of teaching and nonteaching
personnel of the City Schools Division of Dasmarinas, the field is hereby informed that the final date of submission and completion is on October 21, 2022.
2. It is directed that ALL CSDO and school personnel must have submitted their Chest X-ray and Urinalysis results. Hence, those who have failed to comply to the given schedule are required to submit an explanation letter duly signed by their respective school/unit heads.
3. Attached is the list of personnel who have not yet completed the Annual Physical Examination (APE).
4. Immediate dissemination and compliance with this memorandum is directed.
