1. Relative to a letter received from Mr. Jeremiah P. Hari, RSW, Compliance Verification Officer of Provincial Operations Office (POO) – Cavite, this Division informs all public elementary and secondary schools that the field staff of 4Ps in Dasmariñas City will distribute the hard copy of Compliance Verification Forms (CVFs) used in monitoring the educational condition of the program on April 5-15, 2024, on each respective public schools.
2. All 4Ps school focal persons are expected to facilitate the accomplishment of the said CVFs and shall submit the same to the SGOD Office on or before April 18, 2024, 12:00 nn.
