1. Relative to the attached Memorandum OU-LAPSS No. 68 s. 2019 on the Submission of 2018 Preliminary Process for Privacy Impact Assessment, the public elementary and secondary schools and all units in the City Schools Division of Dasmariñas are hereby instructed to accomplish the Data Process Inventory Form (with sample consolidated Data Process Inventory) from the Central Office.

2. The Data Process Inventory Form shall be submitted in soft copy through email to records@depeddasma.edu.ph on or before February 22, 2019.
