1. For the information and guidance of the field, the Health and Nutrition Unit will be conducting the Medical and Dental Health Inspection of Learners for SY 2022-2023.
2. This activity aims to assess the health status of learners, identify their health concerns, if any and provide management or proper referral when necessary.
3. Relative to this, each class adviser shall prepare the students’ medical and dental health forms, and a color-coded expandable envelope which is sorted per grade level. Please refer to the guide below:
Kinder – Red Grade 7 – Pink
Grade 1 – Pink Grade 8 – Yellow
Grade 2 – Yellow Grade 9 – Violet
Grade 3 – Violet Grade 10 – Blue
Grade 4 – Blue Grade 11 – Green
Grade 5 – Green Grade 12 – Orange
Grade 6 – Orange
4. Schedule of the medical and dental health inspection must be strictly followed. Should there be any changes, kindly notify and coordinate with the schools’ respective nurses in-charge in order to make the necessary adjustments.
5. It is strongly reiterated that all symptomatic learners are prohibited from attending face-to-face classes. Furthermore, schools shall ensure that strict compliance with health and safety protocols are implemented.
