1. Pursuant to Regional Memorandum No. 250, s. 2026 titled “Instructions No. 1 for the Implementation of the School-Based Feeding Program FY 2026,” this Division hereby directs all schools to conduct and submit the report on the Baseline Nutritional Assessment of Learners, specifically the height and weight.
2. Reports submitted must be 100% complete. Moreover, schools are instructed to use the BMI Software or School Form 8 (SF8) provided by the Central Office for the recording and consolidation of all assessment results. The assessment shall include the measurement of height and weight using the following indicators:
- Body Mass Index (BMI)
- Height-for-Age (HFA)
3. In this regard, all schools are advised to conduct early weighing of learners in all grade levels upon enrollment to determine their respective nutritional status. To facilitate the conduct of height and weight measurements, the schools may take note the following:
