1. Relative to a letter received from Mr. Vicente T. Tejada, Compliance Verification Officer of Provincial Operations Office (P00) – Cavite, this Division informs all public elementary and secondary schools that Field staff of 4Ps in Dasmariñas City will distribute the hard copy of Compliance Verification Forms (CVFs) which will be used in monitoring the educational condition of the program in the public schools on August 1-4, 2023.
2. All 4Ps school Focal persons are expected to facilitate the accomplishment of
the said CVFs and shall submit the same to the SGOD Office on or before August 8, 2023 (12NN).
