1. Relevant to Division Memorandum No. 209 s. 2025 titled “Annual Physical and Dental Examination of Teaching and Non-Teaching Personnel”, this Division, through the School Health and Nutrition Unit (SHNU), hereby announces the additional schedule for Teaching and Non- Teaching Personnel Annual Physical Examination (APE) for S.Y. 2025-2026.
2. Participants on this activity are teaching and non-teaching personnel who failed to comply their APE within their school previous schedule. All the concerned Teaching and Non- Teaching Personnel are advised to complete their APE requirements from July 2-4, 2025, within the respective date and venue (Please see Enclosure). They should bring all their medical and dental forms, Chest- Xray, Urinalysis and other laboratory results on the said date. Please be reminded that the schedule must be strictly followed.
3. Participation in this activity shall adhere to DepEd Order No. 9, s. 2025 on instituting Measures to Increase Engaged Time-On-Task and Ensuring Compliance Therewith, and strict compliance with other existing DepEd rules and guidelines. Thus, it is expected that School Heads should make the necessary adjustments in compliance thereof.
4. Moreover, all teaching and non-teaching personnel with pending APE requirements will be coordinated by the nurse-in-charge of the division to respective School Heads and Clinic Designate of the school.
