1. Please be informed that the vaccination of the second dose of HPV shall be held on September 8, 2023 at Sta. Cruz Elementary School Library from 9:00 AM to 4:00 PM.
2. All vaccinees are instructed to bring their vaccination card on the said date.
3. On the other hand, division office personnel may proceed to Health and
Nutrition Unit office at Dasmariñas Integrated High School for their second dose.
4. Participation in this activity shall adhere to DepEd Order No. 9, s. 2005 on
Instituting Measures to Increase Engaged Time-On-Task and Ensuring Compliance Therewith, and strict compliance to other existing DepEd rules and guidelines.
