1. In reference to CSC Memorandum Circular No. 05, s. 2026, titled “Adoption of CS Form No. 212, Revised 2026 (Personal Data Sheet),” this Office hereby directs the dissemination of the said Memorandum for the information and guidance of all concerned.
2. The Memorandum prescribes the adoption and use of CS Form No. 212, Revised 2026 (Personal Data Sheet) and the corresponding guidelines for its implementation which can be downloaded from the CSC website.
