1. In line with the assessment of the Nutritional Status of all learners (K-12), the field is hereby directed to submit their report on the baseline nutritional assessment of learners, specifically the height and weight.
2. In this regard, all learners shall undergo Nutritional Status Assessment, particularly utilizing the Body Mass Index (BMI) and Height For Age (HFA). All reports must be 100 % completed.
3. To facilitate the measuring of height and weight, the Dasmaritias City Nutrition Council- City Health Office (Barangay Nutrition Scholars) are tasked to do this activity. All learners must go to the Barangay Health Station (BHS) located nearest to their residence before the enrollment process in the school for the height and weight measurement. This shall be done for the whole month of July until the first week of August. Learners should bring 1/4 sheet of paper with properly named (name, age, grade level, height and weight) for proper documentation. This paper/ form must be attached to Form 137 upon enrollment.
4. Utilize the Body Mass Index Software or the School Form 8 (SF8) provided by the Central Office.
5. All reports must be submitted to School Health and Nutrition Unit at hnu@depeddasma.edu.ph on or before August 26, 2024.
