1. Relative to DepEd Memorandum No. 50, s. 2025 titled “Learners’ Health Assessment and Screening (LHAS)”, the Health and Nutrition Unit hereby informs the field that vision screening for all kinder learners will be conducted by Kindergarten Teachers. Kindergarten learners shall be prioritized for vision screening to facilitate the early detection of visual impairments that may affect learning and development.
2. Vision screening results shall be recorded as “Non-Front Seat” (NFS) or “Front Seat” (FS). A “Non-Front Seat” (NFS) result indicates that the learner met the screening criteria, while a “Front Seat” (FS) result indicates the need for referral to an optometrist or ophthalmologist for further eye examination and evaluation. A “Front Seat” (FS) result is not a medical diagnosis.
3. Learners from Grades 1 to 6, may likewise undergo vision screening, as necessary, in coordination with the Health and Nutrition Unit or the school’s affiliated eye care provider.
4. Learners with a “Front Seat” (FS)” result may be referred to a YAKAP-accredited Optical Clinic for intervention as stipulated in DepEd Memorandum No. 76s. 2025, titled Dissemination of Implementing Rules and Regulations of the Republic Act No. 11358, otherwise known as the National Vision Screening Act.
