Tejada, Compliance Verification Officer of Provincial Operations Office (P00) – Cavite, this Division informs all public elementary and secondary schools that the soft copy of Compliance Verification Forms (CVFs) which will be used in monitoring the educational condition of the program in the public schools shall be accessed through the given link below:
2. All 4Ps school Focal persons are expected to print the soft copy, facilitate the
accomplishment of the said CVFs, and shall submit the same to the SGOD Office on or before October 2, 2023 (12NN).
