1. Relative to a letter received from Mr. Jose Luis F. Decena, Social Welfare Assistant of City Operations Office (C00) – Dasmariñas, this Division informs all public elementary and secondary schools that the Field staff of 4Ps in Dasmarinas City will distribute the Certificate of Compliance (COC) or Retro Forms for Education Condition in a soft copy format through a google link to be sent on the official 4Ps Coordinators group chat.
2. COC or Retro Form is a document used to verify that the monitored children are attending school as required under the program’s education conditionality. Thus, all 4Ps school Focal persons are expected to facilitate the accomplishment of the said form and shall submit the same to the SGOD Office on or before September 3, 2025 (12NN).
