1. The Cavite Educators Lions Club will conduct Sight for Kids Program in the
Division of Dasmariñas. In relation to this, the field is hereby requested to submit the number of students with “Failed Vision”.
2. Failed Vision refers to a student’s decline or loss in the ability of the eyes to see clearly. Signs can include blurred vision, reduced clarity when reading and squinting of the eye that may be caused by various factors such as error of refraction, eye diseases and other health conditions.
3. The report should include the number of recommended students with Failed
Vision per grade level and must be submitted to the Health and Nutrition Unit
