1. This is to inform the field that that the City Schools Division Office, Health and Nutrition Unit is requiring all to submit their Chest X-ray and Urine Analysis results.
2. The final submission will be on May 25, 201 8(Friday), 9AM at the Medical Unit Room of the City Schools Division Office.
3. Attached in this memorandum is the list of Teaching and Non- Teaching Personnel with no chest x-ray and urinalysis results.
