1. Relevant to DM-OUOPS-2024-03-06789 titled Interim Guidelines on the Resumption of School-Based Immunization (SBI) Program after the COVID-19 Pandemic, all public elementary and secondary schools are hereby informed of the resumption of the conduct of school-based immunization this October 2024. This program aims to provide protection against vaccine-preventable diseases such as measles, rubella, tetanus, diphtheria (MR-Td), and human papillomavirus (HPV).

2. Relative to this, all schools are instructed to distribute parental consent forms to all male and female Grade 1 and Grade 7 pupils for the MR and Td vaccination, and to all female Grade 4 pupils for the HPV vaccination. These must be secured prior to the conduct of the immunization.

3. In addition, clinic teachers of each school must accomplish Recording Forms 1, 2, and 3 for Grade 1, Grade 7, and Grade 4 learners respectively. These forms must be submitted to the City Health Office staff on the day of the scheduled vaccination. Attached in this memorandum are the copies of the consent form and recording forms.

4. School heads are advised to make necessary arrangement to ensure that there will be no disruption of classes as stipulated in DepEd Order No. 9 s. 2005 entitled Instituting Measure to Increase Engaged Time-On-Task and Ensuring Compliance therewith during the conduct of this activity.

5. The Interim Guidelines on the Resumption of School-Based Immunization after the COVID-19 Pandemic are contained in the Enclosure.

6. Widest dissemination of and strict compliance with this Memorandum is directed.
