1. In compliance with Section 8 of Republic Act No. 6713, the “Code of Conduct and Ethical Standards for Public Officials and Employees”, the field is directed to submit the STATEMENT OF ASSETS, LIABILITIES & NET WORTH (SALN) as of December 31, 2018 in electronic copies and two original copies together with the Summary List of Filers (template attached) on or before April 30, 2019.
2. The electronic copies of the SALNs must be in PDF format and individually saved per declarant, in compact discs (CD) or Universal Serial Bus (USB) flash drive. The file name of the soft copy must be the same as the document title.
