Investigation on September 16, 2019 to December 6, 2019 among principals I officer-in-charge of particular schools relative to the construction of school-based barangay health stations for SY 2014-2015 and 2015-2016, this Office provides the attached list of public elementary school heads who are required to accomplish to the best of their knowledge the questionnaires (Annex A) provided by the Office of the Commission on Audit – Special Services Sector.
Moreover, the filled out forms together with the Sworn Statement shall be sent directly to the Special Audit Team addressed to Honey Pretzel Tolentino, 3 Floor, Fraud Audit Office, Commission on Audit, Commonwealth Avenue, Quezon City 0880.
