This is to inform the field that as per advice of the Health and Nutrition, Regional Office, the Schools are hereby requested to implement Rabies Awareness Program on September 28, 2016. This is based on the instruction and lectures given last September 19, 2016 (Rabies Awareness and Food Safety Roll-Out DM dated September 7, 2016) at the AVR Congressional National High School.
Suggested Activities to be done are the following:
a. Film Showing
b. Posting of Posters
c. Giving Flyers
d. Information Dissemination through Lecture/Health Talk
The Schools are required to document all the activities to be done and a narrative report will be submitted to the Health and Nutrition Unit, City Schools Division Office on October 3, 2016.
In this regard, for the uniformity of the report, attached herewith is the narrative report form.
