For the information and guidance of all concerned, attached is a copy of DM-PFO-2018-0714 from the office of Undersecretary Jesus L. R. Mateo entitled ” Clarification on Requirement of Birth Certificate for the Purposes of Enrolment in DepEd Public Schools” which is self- explanatory.
School Heads are required to disseminate this information during faculty/teachers meetings and parents’ orientation including posting of information bulletins on the SDO/SCHOOL website.
In relation, a narrative report relative to the dissemination efforts conducted in all schools shall be submitted to this Office c/o SGOD on July 20, 2018. The reports shall be made as integral part of the Division narrative report that will be submitted to the regional office.
Immediate dissemination and compliance to this Memorandum is directed.
