With reference to Division Memorandum No. 184, s. 2022 Re: Submission of Office Performance Commitment and Review Form (OPCRF) And Individual Performance Commitment and Review Form (IPCRF) for the School Year 2021-2022, the field is hereby informed of the following revised guidelines:
1. DepEd Dasma is obliged to comply with the Data Privacy Act of 2012. Thus, the uploading of IPCRF/ OPCRF will be via Google Drive Folders.
2. One (1) school uploader must be assigned by the school head.
3. The official school uploader must fill out the link https://forms.gle/U9f5Gbsi843Q6RuD9 or bit.ly/2022IPCRFuploader
4. Unique link specific to the designated school will be sent to the school official uploader email once the uploader has filled out the provided e-form.
5. Once the link has been accessed, upload the files in portable document format (pdf) ONLY.
