In line with the DepEd Memorandum dated September 17, 2018 re: Data Gathering on the Number of DepEd Employees with Disability, all concerned personnel are directed to participate in the nationwide data gathering procedure by accomplishing the attached survey form (Annex A: Survey for Employees with Disability) and submit the accomplished survey form on or before October 10, 2018 to Personnel Unit.
Immediate dissemination and compliance of this Memorandum is directed.
