In compliance to the Annual Physical Examination (APE) of teaching and non-teaching personnel of the City Schools Division of Dasmariñas, the field is hereby informed that the final date of submission and completion is on May 30, 2019, 9:00 AM at the CSDO Medical Office.
It is directed that ALL CSDO and school personnel must have undergone the medical and dental examination in addition to their submitted Chest X-ray and Urinalysis results. Hence, those who have failed to comply to the given schedule are required to submit an explanation letter duly signed by their
respective school/unit heads.
Attached is the list of personnel who have not yet completed the Annual Physical Examination (APE).
