This is to inform the field that the City Schools Division of Dasmarinas – Health and Nutrition Unit is requiring all concerned to submit their Chest X-ray and Urinalysis results.
The final submission will be on June 8, 2018, Friday, 9:00 AM, at the Medical Unit.
Failure to submit on June 8, 2018 will require the school head to submit an explanation letter as to why they have still failed to comply on the given date.
Attached in this Memorandum is the list of teaching and non- teaching personnel with no chest x-ray and urinalysis results.
