In response to a letter from the Government Service Insurance System (GSIS) regarding teaching and non-teaching personnel from the City Schools Division of Dasmariñas who were already issued Business Partner (BP) Numbers but have not yet enrolled in the UMID Card facility, this office requested the GSIS for an on-site UMID Card Enrollment tentatively scheduled on the last week of June at the Schools Division Office.

All concerned personnel in the attached list from GSIS are advised to accomplish the GSIS UMID-eCard Enrollment Form and GSIS eCard Customer Information Record. The accomplished forms together with a photocopy of two (2) valid IDs and a transmittal letter signed by School Head consisting the list of enrolling members shall be submitted to the Records Unit on or before June 11, 2019.
