This office directs all schools to adopt the new template for the Recommendation of Appointee effective immediately. The new recommendation template shall be used in the submission of the school’s prospective appointee prior to the issuance of the Notice of Document Submission. The said recommendation shall likewise be used as part of the documentary requirements for appointment.
Attached to this memorandum is the template of the recommendation to be used with the school’s official letter head.
