Authority is hereby granted to Mr. Cesar Chester 0. Relleve, Education Program Supervisor and Ms. Melanie S. Pamienta, Planning Officer III, to conduct a meeting on January 24, 2020, Friday at the Bulwagang Sigasig. Participants to this meeting are the following:
1. Dr. Gemma G. Cortez, CID Chief
2. Education Program Supervisors
3. Public Schools District Supervisors
4. School Checking Committee (SCC) Chair and the two Vice Chairs, as per Deped Order 11 s. 2018, SCC is composed of
4.1 School Head
4.2 LIS / ICT Coordinator or Senior High School Registrar
4.3 School Personnel (Curriculum and Assessment)
Agenda are as follows:
a. Guidelines on the Preparation and Checking of School Forms
b. Other Matters.
The meeting will be conducted in two separate sessions: Morning Session, 9:00 AM to 12:00 NN – Participants from Elementary School Afternoon Session, 1:00 PM to 4:00 PM – Participants from Junior and Senior High Schools Supervisors may either attend in the morning or in the afternoon session. Participants are encouraged to bring a sample of last school year School Forms Checking Report (SFCR1) and other related sample of school forms that could be sourced of SCC inquiry and clarification.
