The field is hereby informed of the attached Regional Memorandum (RM-19-330) entitled Nutritional Assessment of All Learners (Baseline) for SY 2019-2020.
It is advised that all schools must prepare for the monitoring of height and weight measurement of learners come June 2019.
Relevant to this, schools are directed to use and download the School Form 8 Learner’s Basic Health and Nutrition Report at http://bit.ly/SchoolForm8 . Accomplished school forms must be submitted to the official Health and Nutrition Unit email at hnu@depeddsma.edu.ph on or before June 28, 2019.
