Relative to OUA Memorandum 00-0222-0054 titled Online Survey on the Training Needs of Student Leaders and Teacher-Advisers dated February 10, 2022, the field is hereby informed of the conduct of the online training needs assessment survey of the Bureau of Learner Support Services – Youth Formation Division. The deadline of submission for the accomplished survey forms is on February 28, 2022.
