Relative to Division Memorandum No. 126, s. 2021 re: Online Health Assessment and Submission of Chest X-Ray and Urinalysis Results of Teaching and Non-Teaching Personnel, the field is hereby informed that the conduct of the Annual Physical Examination (APE) is suspended due to incidences of Delta Variant in the Province of Cavite. Further notice on the new schedule will be issued in another Memorandum.
