Relative to the coordination meeting held last January 7, 2022 headed by Hon. Jennifer Austria-Barzaga, City Mayor, all public and private schools in this Division are instructed to facilitate the pre-registration for pediatric vaccination of all their learners willing to be vaccinated, aged 5 to 11 years old, while waiting for the final guidelines and advisory from the Department of Health.
Attached is the template of the master list that must be accomplished by each school and must be submitted via email to hnu@depeddasma.edu.ph on or before January 21, 2022. The subject of the email must follow the format below:
NAME OF SCHOOL – PEDIA VACCINATION 5 to 11 YEARS OLD
For strict compliance and immediate dissemination.
