Relative to Division Memorandum No. 126, s. 2021 re: Online Health Assessment and Submission of Chest X-Ray and Urinalysis Results of Teaching and Non-Teaching Personnel that was postponed last July 27, 2021, the field is hereby informed that the conduct of the Annual Physical Examination (APE) may be continued in November 2021 onwards but schools with laboratory exam to be conducted this October may proceed with their schedule.
