This is to inform all public elementary schools that the City Health Office of the City of Dasmarinas will conduct school-based immunization of Measles-Rubella. This program aims to immunize school children to protect and prevent the occurrence and transmission of Measles and Rubella disease among them.
Relative to this activity, all schools are instructed to distribute parental consent forms to all Grade 1 pupils aged 6 years old to 7 years old and Grade 7 pupils aged 12 years old to 13 years old. Parental consent forms must be secured prior to the conduct of the immunization.
In addition, clinic teachers at each school must accomplish Recording Forms 1 and 2 for Grade 1 and Grade 7 learners respectively which must be submitted to the City Health Office staff on the day of the scheduled vaccination. Attached in this memorandum are the copies of the consent form and recording forms.
School heads are advised to make necessary arrangement to ensure that there will be no disruption of classes as stipulated in DepEd Order No. 9 s. 2005 entitled Instituting Measure to Increase Engaged Time-On-Task and Ensuring Compliance therewith during the conduct of this activity.
