| Under Permit Status (schools who apply New / Renewal Permit before opening of School Year) | SEC Registration Articles of Incorporation |
| With Government Recognition Permit (schools who are) | SEC Registration Articles of Incorporation Government Recognition Certificate / Permit |
| Senior High School with Provisional Permit to Operate | SEC Registration Articles of Incorporation |
