Pursuant to DepEd Memorandum No. 59, s. 2015 (Change In and Additional Information to the Submission of School-Based Child Protection and Anti-Bullying Policies and Reports on Compliance) and DepEd Memorandum DM-LA-2016-0153 of the Office of the Undersecretary for Legal and Legislative Affairs dated June 20, 2016, all school principals and teacher-in-charge of public and private elementary and secondary schools of Dasmariñas City are directed to submit the following reports to the Legal Unit of the City Schools Division of Dasmariñas City on or before July 11, 2016:
a. School-Based Report on Incidents of Bullying (SY 2015-2016)
b. School-Based Report on Incidents of Child Abuse (SY 2015-2016)
Attached herewith as Annex “A” and “B” are the sample forms of School-Based Report on Incidents of Bullying and School-Based Report on Incidents of Child Abuse, as enclosed in DepEd Memorandum No. 59, s. 2015. The forms may also be downloaded from www.deped.gov.ph as enclosures to the said DepEd Memorandum.
A report should still be submitted even if no incident of bullying or child abuse occurred in the school for SY 2015-2016 with an indication that no such incident was reported for the given school year.
