Pursuant to Memorandum No. 61, s. 2019 issued by DepEd Undersecretary Tonisito M.C. Umali, Esq., all public and private schools are directed to submit the following reports to the Legal Unit of the City Schools Division of Dasmariñas on or before February 15, 2019:
a. School-Based Report on Incidents of Bullying (SY 2018-2019)
b. School-Based Report on Incidents of Child Abuse (SY 2018-2019)
C. Updated Child Protection and Anti-Bullying Policy in accordance with RA 10627 (Anti-Bullying Act of 2015)
The baseline information will be the basis of the Central Office in crafting anti-bullying programs.
Attached herewith as Annex “A” and “B” are the sample forms of School-Based Report on Incidents of Bullying and School-Based Report on Incidents of Child Abuse respectively, as enclosed in DepEd Memorandum No. 59, s. 2015.
A report should still be submitted even if no incident of bullying or child abuse occurred in the school for SY 2018-2019 with an indication that no such incident was reported for the given school year.
