Pursuant to DepEd Memorandum No. 114, s. 2017, Reiteration of the Reportorial Obligation of Schools, Schools Division Offices, and Regional Offices pursuant to DepEd Order Nos. 40, s. 2012, 55, s. 2013, and 18, s. 2015, all public and private schools are directed to submit the following reports
through the official email of DepEd Dasmarifias Legal Unit: legal@depeddasma.edu.ph on or before July 06, 2020, 5:00 PM:
a. School-Based Report on Incidents of Bullying (SY 2019-2020)
b. School Based Report on Incidents of Child Abuse (SY 2019-2020)
c. School-Based Report on Cases of Children-At-Risk (CAR) (SY 2019-2020)
d. School-Based Report on Cases of Children in Conflict with the Law (CICL) (SY 2019-2020)
Attached herewith as Annex “A”, “B”, “C”, and “D” are the report templates for cases of Bullying, Child Abuse, CAR and CICL respectively.
A report should still be submitted even if no cases occurred in the school for SY 2019-2020 with an indication that no such incident was reported for the given school year.
