On the basis of Regional Memorandum 08D-17-357, this Office is requesting all schools to submit the following documents on an annual basis.
1. Enrollment list of students/ pupils within sixty(60) days after the opening of classes (please refer to the table below);
2. List of graduates and all scholastic records pertinent thereto, not later than thirty (30) days from the date of graduation (please refer to the table below).
In this regard, for this year the following documents shall be submitted to this Office on or before August 7, 2017….
