This is to remind the field that the City Schools Division of Dasmarinas – Health and Nutrition Unit set the deadline of submission of Chest X-ray and Urinalysis results last June 8, 2018. However, there are still some who failed to comply on the given date. For this reason, all school heads are required to
submit an explanation letter addressed to the Schools Division Superintendent stating the reason of non-compliance as stated in Unnumbered Memorandum dated May 30, 2018 with the subject: Final Submission of Diagnostic Exam Results of Chest X-Ray and Urine Analysis.
The deadline of submission will be on June 22, 2018, Friday, at the Medical Unit.
Attached in this Memorandum is the list of personnel with no chest x-ray and urinalysis results.
