The field is hereby requested to submit the following reports in soft and in hard copy:
a. List of Teachers by Teaching Assignment and Area of Specialization (additional column PRC LICENSE)
b. List of Personnel and Their Religious Affiliations
c. List of Personnel Who Have Undergone Minor / Major Operation for the Last Three (3) Years
Submission of the enclosed reporting template is on or before January 15, 2020, whereas reports must be submitted in excel format and thru email at planning@depeddasma.edu.ph.
