This is to inform the field that the office of Private Schools will be receiving only the LIS-Generated School Form 5 for School Year 2016-2017. The School Form 6 will be manually encoded in the downloadable form. The forms that will be accepted is in landscape format. Deadline of submission is until April 28, 2017 (Friday) only. No extension will be given for proper checking of
the said documents.
