In line with the Guidelines on the School-to-School Partnership for Fiscal Year 2016, this office requires all the respective Leader schools to submit liquidation reports in Excel format, based on Regional Memo No. 060, s.201 7 on February 8,
2017 at 1:00PM.
The report should include the following:
a. Report on Fund Utilization and Liquidation (Annex 1 0)
b. Accomplishment Report Template (Annex 11)
Submit the hard copy to the SGOD attention; Vic Villador, SEPS-M&E, while the soft copy shall be emailed at vic.villador@deped.qov.ph
