1. Relative to the letter received from Mr. Jeremiah P. Hari, RSW, Compliance
Verification Officer of Provincial Operations Office (P00) – Cavite, this Division
informs all public elementary and secondary schools that the Field staff of 4Ps
in Dasmariñas City will distribute the hard copy of Compliance Verification (CV)-
Retro Forms used in monitoring the educational condition of the program on
January 18, 2024, on each respective public schools.
2. All 4Ps school Focal persons are expected to facilitate the accomplishment of the said CV-Retro Forms and shall submit the same to the SGOD Office on or before January 24, 2024 (12NN).
