1. Relative to the Division Memorandum No. 042, s. 2023 titled “Request for Research Indorsement”, this Office reiterates that the requesting party shall submit the following requirements for the conduct of their research/study:
a. Letter request to SDS in conducting the study/data gathering;
b. Research Tool/Instrument;
c. Letter of Consent to the participants of the study; and
d. Notarized Memorandum of Agreement for data privacy and data sharing document.
2. Enclosed with this Memorandum is the updated Memorandum of Agreement (MOA) for data privacy and data sharing, along with the process to request permission to conduct research in the Division.
